Services for My Patient
We work with physicians, doctors, hospitals, and rehabilitation centers on a daily basis. Assisting people who are referred to us is a primary function of Forte Health Services. The 4 steps below give a basic overview of the process when a medical professional refers their patient.
STEP 1
Referral sent to Forte Health Services
We will receive and process the patient referral sent from your physician’s office.
STEP 2
Scheduled appointment with a registered nurse (RN)
We will call the patient directly to schedule a date and time for an RN to visit the home.
STEP 3
Approval
Our office will submit an order and paperwork to your physician for approval and development of the patient’s plan of care.
STEP 4
Admit
With the approval of the physician, our nurses will begin meeting with the patient on the scheduled dates.
