Do I Qualify for Home Health Services?
In general, there are a few requirements that you must meet to qualify for home health care from Forte Health Services.
You likely will qualify if you are:
- Homebound, according to your doctor’s certification.
- In need of skilled nursing care or continued therapy.
- Under the care of a doctor and receiving services under an established plan of care that is reviewed by your doctor.
